What is the Excellence in Care Award? It is a very attractive framed Award with the name of the Award Winner and a statement of appreciation from LMSarcoma Direct Research Foundation. The award also lists the names of all the people who nominated the awarded individual or institution. A photo of this very attractive award will be posted on this site at a later date. It is something anyone would be proud to hang on their office wall, because it symbolizes our deepest thanks for their dedication.
Many of you have told us of a doctor, nurse, technician, hospital or other medical personnel who has provided extraordinary care for you or a loved one. LMSarcoma Direct Research Foundation has created this Award as a way to help you honor these special people.
Your nominated honoree will receive a personal letter notifying them they have been nominated for the award. This letter will tell the nominee the name of the person nominating them for this Award. The nominees will all be featured in our Newsletter (first edition anticipated this fall) and nominee names will be shown on this website page.
Nominations for 2007 will end on December 31st of 2007 and the 2007 Award will be presented in the month of January, 2008.
To participate, simply send us a note (P.O. Box 52697, Tulsa, Oklahoma 74152) with your donation of $50 or more telling us who you are honoring and why they deserve the recognition. Be sure to identify the honoree by name, address, and phone number so we can send them notification that they have been nominated for this award and who has placed their name into nomination. You may place more than one vote for your nominee. Example: If you send a check for $200 you will give your nominee a total of 4 votes. The Award will be based on the number of nominations/votes for each honoree, and upon a confirmation by the Board of Trustees that this nominee truly merits the award.
